Case Management

The Case Management Department uses a team of nurses and social workers who assess, plan, coordinate and monitor services received by our patients, in an effort to provide an appropriate discharge plan. The team evaluates all options for each patient, and works closely with the physician and family members to assure that proper services will be in place at the time of discharge.
